Do You Need Copies of Medical Records?
If you need copies of your medical records or a program attendance letter, please review our policies for Obtaining Copies of Your Medical Records. There is no charge for a program attendance letter; however, you may be charged for copies of your medical records based on the number of pages copied.
Request for Medical Records Process
- Open and print the Authorization to Release Information.
- Complete and sign the Authorization to Release Information. Please note: The signature on the release must match the original signature in your medical record.
- Return the form with “original signature” by U.S. Mail or other service such as Fedex. We do not accept faxed releases.
